Horse dying from castration

I had the vet out to check my horse out and give him his shots and castrate him.
The vet told me after he had castrated my horse that Spotted Saddle horses have a tendency for their intestines to come out.
The next morning my horse was dead with his intestines hanging out.
I have done some research and if he had done the procedure properly this would not have happened.
I was not given the opportunity to make an informed decision for the procedure and would not have had Apache castrated.

How often does this happen?
