nipple caking on a cockapoo

My nine year old cockapoo has very sensitive pink skin under her white coat. She is constantly itching. I've tried different foods, shampoos, bathing schedules and nothing helps.

Now, I've noticed caking and discoloration around two of her nipples. She is always scratching that area.

What can I do?
